On the grounds of MacLaren Youth Correctional Facility in Woodburn. Interim Oregon Youth Authority Director Jana McLellan on Thursday described her “back to basics” efforts to stem the crisis that has swamped the state agency since February, when The Oregonian/OregonLive first reported that the chief investigator of the Professional Standards Office had abruptly resigned over his handling of complaints by youths, including allegations about sex abuse by staff.

The unit within the Oregon Youth Authority responsible for investigating complaints failed to adequately track them by type, triage them based on urgency or flag employees whose names appeared multiple times, agency leaders told lawmakers Thursday.
