Employers are bracing for their biggest healthcare cost increase in more than a decade, a new survey finds. Workers could feel the squeeze in their premiums, deductibles and paychecks.

When Matthew Johnson took over a small electrical contracting business in Portland this spring, one expense quickly caught his attention: healthcare.
